Shape the Future of Health in Livingston County: Register for a CHNA Focus Group

Your voice matters.

Trinity Health Livingston and the Livingston County Health Department (collaborating as the Liv.Well Planning Team) are currently gathering community insights for the upcoming Community Health Needs Assessment (CHNA). If you live or work in Livingston County, your unique perspective is needed to help determine where our community should invest its time, talent, and resources over the next few years.


What is the CHNA?

The CHNA is an assessment conducted every three years by non-profit hospitals to identify local health and social needs. By partnering with community members and local organizations, the goal is to develop real, actionable strategies that improve access to care, address health equity, and enhance overall well-being in our neighborhoods.

By participating in a focus group, your feedback will directly influence the area’s upcoming Community Health Needs Assessment Implementation Plan.


Choose a Focus Group Session

Two separate focus group opportunities are being offered in June. Please only register for one session, as space is limited to the first 40 registrants per group.

Option 1: Hartland Session

  • When: Friday, June 19, 2026
  • Time: 3:30 PM – 5:00 PM
  • Where: Cromaine District Library
    3688 Hartland Rd, Hartland, MI 48353

Option 2: Fowlerville Session

  • When: Friday, June 26, 2026
  • Time: 2:30 PM – 4:00 PM
  • Where: Fowlerville District Library
    130 South Grand Ave., Fowlerville, MI 48836
